Applying to become a Provider

This online application is used by persons or entities desiring to obtain approval of the Florida Board of Funeral, Cemetery and Consumer Services as a continuing education provider under Chapter 497, Florida Statutes. Once an entity becomes an approved provider, each course offering must be approved by the Board.
