- Already licensed with the Division of Funeral, Cemetery and Consumer Services, but this your first visit to our site.
|Providers are required to submit Course (with Instructor) applications to the Division through the online application process. Roster submissions are made by simply entering the course attendees directly into the system or by submitting the attendance roster online via a file attachment process. There is a $200 provider application fee and $5 unlicensed activity fee.|
Written verification of all application approvals will be replaced by an online In-Box notification process. Additionally, during the review process, the Division can request additional information online, allowing the provider to respond to the additional information requested.
Non-Licensees and Potential Providers
|Individuals seeking information on courses can search for courses by clicking on the Locate menu at the top of the page. Non-licensees can also view continuing education requirements for a particular license type by clicking on the Locate menu at the top of the page.|
Potential providers can apply for initial approval online. Click on the Apply menu at the top of the page.
